spatial index creation from another software 2012

ARCGIS software needs to create spatial index but it is not allowing the creation. The add button in arcgis software is greyed out? Connects to sql2012 and sees data. Thoughts.

does the table have a clustered primary key? can you create a spacial index with TSQL (is the problem limited to arcGIS)?
