perfmon counters benchmarks

I am looking for perfmon counters usages for CPU,MEMORY and I/O subsystem performance related thresholds and automated script for sending alerts to appropriate teams for the same. Especially for Memory(RAM) utilization issue, need proactive alerts before it struggles.

While you can certainly roll your own, you might take a look at 3rd party tools like SQL Sentry, Idera's Diagnostic Manager, or Red Gates SQL Monitor to help facilitate those alerts. Just an idea.
