How to transform this table data into different column alias?

How can i transorm this table.. ![alt text][1] into this form using an SQL querry. ![alt text][2] I am using SQL server 2005 and need to transform the data into diff columns for Stamping values to pdf's dynamically. [1]: /storage/temp/1083-\capturesql2.png [2]: /storage/temp/1084-\capturesql3.png
