Do you mean a model of the system tables? Or are you looking for tools to allow you to create models of your databases?

If you are after some extra information on system tables - that's the current topic I'm writing about on my blog - http://www.atlantis-interactive.co.uk/blog/default.aspx - although not diagrammatic, it does have some extra information, explaining where MSDN is misleading or just plain wrong.

I've used both CA ER-Win and ModelRight.

I suppose you are asking about how to go about modelling your database,for a simple db, you can start with database diagrams in sql server, and if it is large then go for something like erwin from ca or Embarcadero studio or any other tool that you like.

Sybase PowerDesigner, CA ER-Win Data Modeler, Embarcadero ER Studio, Microsoft Visio

Microsoft Visio is simple one and nice job. I use this for modelling and reversed engineered to get diagrams of existing database.
